PTAG Standard Application for Approval of Continuing Competency Hours(CCHs) Courses

PTAG Application for Approval of Georgia Ethics & Jurisprudence Course CCHs
Complete one copy of the application to be returned via mail, courier or email (firstname.lastname@example.org) with all submission items included! - CEU Designation Change:Going forward from this date CEUs will now be referred to as Continuing Competency Hours (CCHs) and will no longer institute the conversion from Contact Hours to CEUs: ( 1 Contact Hour = .1 CEUs)
From now on 1 Contact hour will = 1 Competency Hour or 1 CCHs
- PTAG approval of a course is approved for one calender year and does not guarantee acceptance by the Georgia State Board of Physical Therapy for continuing education credits required for relicensure. See Georgia State Board of Physical Therapy Rule 490-4-.02 and Policy #7.
- CCH Application Fee Change: Effective October 16, 2014, CCH application fees have changed. Please refer to the form for the new fees.
- CCH Renewals Policy Change: Beginning September 1, 2010, CCH providers wanting to renew a course will be required to submit the CCH application with the appropriate documents and required fee. PTAG can no longer auto-renew courses with a letter of request. If you have any questions regarding this process, email the PTAG Executive Office.

Each participant must sign this roster at the beginning and again at the end of the course. Sponsor must verify attendance, enter CCH’s approved and return original roster to PTAG at the above address (mailing or email) within two weeks of the program (please keep a copy for your records) in order for PTAG to maintain a record of attendance for each participant.

PTAG provides a list of APPROVED CONTINUING COMPETENCY COURSES. If your course was approved and it is not listed, please email the PTAG Executive Office. To search, open the PDF and use Control + F on your keyboard. This will begin the search inside the file.
